Differences between the Traditional Survey and the QIS
The QIS is indeed a departure from what we’re accustomed to in the “traditional survey”. Here are some of the main differences:
Traditional Survey
QIS Survey
Off –site preparation and sample selection
Surveyors review OSCAR data, recent complaints and QM/QI reports to identify sample of residents and areas of concern prior to survey
Surveyors review OSCAR data, recent complaints, download MDS data to tablet PCs and use software for resident sample selection
Entrance Information
Review of Roster Sample Matrix Form (CMS 802)
Surveyors obtain alphabetical resident census with room numbers and units and a list of new admissions over the last 30 days
Survey Structure
Resident sample is approx. 20% of the facility census for resident observations, interviews, and record reviews. Phase I consists of focused and comprehensive reviews based on QM/QI data. Phase II consists of focused record reviews. Facility and environmental tasks are also completed during the survey
Stage I consists of preliminary investigation of regulatory areas in the admission and census samples. Stage II consists of completion of in-depth investigation of triggered care areas and/or facility level tasks based on Stage I findings
Group Interview
Surveyors meet with the Resident Council and use council minutes to identify concerns
Surveyors interview the Resident Council President or Representative
